Liability Waiver

Program Structure Acknowledgment

I understand that Mauch Art Studio offers structured, instructor-led classes and workshops with defined session times and is not a childcare facility. I understand that participation is for educational and enrichment purposes focused on arts, science, writing, and creative learning experiences. This program is not licensed or registered with the Texas Health and Human Services Commission Child Care Regulation.

Assumption of Risk

By registering my child for a program, class, or camp hosted by Mauch Art Studio, I acknowledge that participation in creative and educational activities may involve certain inherent risks. These activities may include, but are not limited to, the use of art materials, craft tools, science experiment supplies, sensory materials, movement activities, and group work.

Safety & Supervision

I understand that while Mauch Art Studio takes reasonable precautions to provide a safe environment and appropriate supervision during class sessions, accidents and injuries may occur. I voluntarily assume all risks associated with my child’s participation.

Medical Authorization

In the event of an illness or injury requiring medical attention, I authorize Mauch Art Studio staff to obtain appropriate medical care for my child if I cannot be reached. I understand that reasonable efforts will be made to contact me or the emergency contact listed during registration.

I agree to disclose any known medical conditions, allergies, behavioral needs, or other relevant health information so staff can best support my child’s safety and participation.

Behavior & Participation Expectations

I understand that Mauch Art Studio strives to maintain a safe, respectful, and positive environment for all participants. If a child’s behavior is unsafe or repeatedly disruptive, the studio reserves the right to dismiss the child from the program. Refunds may not be issued in such cases.

Release of Liability

I hereby release, waive, and hold harmless Mauch Art Studio, its owner, employees, assistants, and volunteers from any claims, liabilities, damages, or expenses arising from my child’s participation in studio programs or activities, except in cases of gross negligence.

Media Release

​I grant permission for photographs or videos taken during studio activities to be used by Mauch Art Studio for promotional purposes, including social media, marketing materials, and the studio website, unless I notify the studio in writing otherwise.
