Participant Liability Waiver
By registering my child for a program, class, or camp hosted by Mauch Art Studio, I acknowledge that participation in creative and educational activities may involve certain inherent risks. These activities may include, but are not limited to, the use of art materials, craft tools, science experiment supplies, sensory materials, movement activities, and group play.
I understand that while Mauch Art Studio takes reasonable precautions to provide a safe environment and appropriate supervision, accidents and injuries can occur. I voluntarily assume all risks associated with my child's participation in these activities.
I hereby release, waive, and hold harmless Mauch Art Studio, its owner, employees, assistants, and volunteers from any claims, liabilities, damages, or expenses arising from my child's participation in studio programs or activities, except in cases of gross negligence.
Medical Authorization
In the event of an illness or injury requiring medical attention, I authorize Mauch Art Studio staff to obtain appropriate medical care for my child if I cannot be reached. I understand that reasonable efforts will be made to contact me or the emergency contact listed in the registration form.
Allergy & Health Disclosure
I agree to disclose any known medical conditions, allergies, behavioral needs, or other relevant health information during registration so staff can best support my child’s safety and participation.
Behavior Expectations
I understand that Mauch Art Studio strives to maintain a safe, respectful, and positive environment for all campers. If a child's behavior is unsafe or repeatedly disruptive, the studio reserves the right to dismiss the child from the program. In such cases, refunds may not be issued.
Photo & Media Permission
I grant permission for photographs or videos taken during camp activities to be used by Mauch Art Studio for promotional purposes, including social media, marketing materials, and the studio website, unless I notify the studio in writing otherwise.
